호주 인턴 일기 3 - Guild Workshop 1 정리

사이클론 기간에 진행했던 첫 번째 Guild 워크숍 1 (3시간 분량) 정리 포스팅.
*내용은 강의 중에 받아적으면서 정리한 것이어서 다소 두서없을 수 있음 *
 

1. AHPRA 인턴 필수 요건

- ITP Completion Certificate (ITP 수료증)
- Supervised Practice Hours  1,575시간 (2025년 기준)
- APC Written Exam & PBA Oral Exam 합격
 

2. ITP 주요 일정 및 제출 기한

  

1) The College ITP Coursework (과제 기한은 권고 날짜임)

* Submission 1 (2025년 3월 31일 마감)
- CPD Learning plan
- MCQ modules 10개
- Entrustable Professional Activities
- Week 9 Intern Preceptor Assessment
 
* Submission 2 (2025년 6월 23일 마감)
- Week 18 Intern Preceptor Assessment 포함 기타 등등
 
*Submission 3  (2025년 10월 20일 마감)
- Intern Portfolio 포함 기타 등등
 

2) 필수 참석 세션

- Workshop 1 (1/2 day, 오늘 들음)
- Workshop 2 (1 day, 하반기 예정)
 

3. Intern Exam 개요

- Written exam & Oram exam 연 3회 (2월, 6월, 10월)
- 시험 응시 전 Supervised hours의 75% (1181시간) 충족 필수
- 시험 등록 기간(Registration period) 내 75% 시간 충족 못했어도 신청 가능 -> 시험일 전까지 75% 충족하면 응시 가능

2025 Intern exam dates

 

1) Written Exam (APC)

- 75문항 MCQ (2시간) 
- 67문항 점수 반영, 8문항은 점수 미반영(실험용 문제) 이지만 어떤 문항이 점수 반영 문항이고 어떤 문항이 점수 미반영인지알 수 없음
- Computer Based Exam
- 오픈북 시험 (AMH, APF, PBS, 기타 인쇄물 및 필기노트 허용)
- 비용: $770
- 준비물: 여권 또는 호주 운전면허증
- 범위: 6 content areas based on the National Competency Standards Framework for Pharmacists in Australia 2016     (https://www.pharmacycouncil.org.au/resources/intern-written-exam-guide-and- sample-paper/ )
- written exam은 Pearson Vue program 을 이용해서 진행되기 때문에 사전에 집에서 프로그램 미리 설치하고 익숙해지면좋음(https://www.pearsonvue.com/us/en/apc.html)
- 헷갈리는 문제 있으면 Flag 표시해두고 마지막에 돌아와서 다시 풀 수 있음
 

2) Oral exam (PBA)

- 시험 2 주 전에 정확한 시험 일자 안내 메일 보내줌
- 시험 전 APOE-60 form 제출 해야함 (마감기한 절대엄수) (https://www.ahpra.gov.au/Registratio n/Online-upload.aspx)
- 비용: $475
- Written 시험은 national based 라서 어느 주에서 보든 상관 없지만, Oral 시험은 state based라서 인턴십 수행한 주(state)에서 응시하는 게 좋음 (oral 시험 part2가 legal &ethical practice인데 주마다 legal requirement 다르기 때문)
- 각 방마다 채점관 2명, oral 시험은 모든 파트(A,B,C) 합격 필수 (한 파트라도 불합격시 전체 낙제)

4. Oral Exam  세부 내용

  

1) PartA: Primary Healthcare (OTC상담)

- Reference 사용 불가
- 환자 히스토리 테이킹 필수 (e.g. Is it for yourself? Any allergies? On other medications? CAM?)
- Contraindication, Red flag 확인 (e.g. Pregnant constipation 환자에게 stimulant laxative가 best option은 아님)
- Dose, Strength (e.g. Naproxen 275mg), Direction (e.g. apply Dermaid 1% cream  2-4 times a day up to 7 days) 도 제시해야함
- 시험 기간이 10분 뿐이라 Drug interaction을 찾아야 할 확률은 낮음 (전혀 없다는 건 아님. 뭐 어쩌라고..?)
- 케이스 다양. Fungal infection 케이스일 수도 있지만 그냥 I hit my head on car door. What should I do? 손님처럼 simple할수도 있음. Refer 해야하는 상황 있을 수도 있음 (Refer point 언급해주기, 예를 들어 If this doesn't get better within 7 days, you need to see doctor)
- Brand name 말해도 되고, Active ingredient 말해도 됨 (e.g. Nexium - Esomeprazole)
- Life style advice 마지막에 한 두개 꼭 언급해주기
 

2) Part B : Legal and Ethical practice

- Common sense 이용하기(없으면요..?ㅠ)
- Legal part 와 Ethical part 둘다 고려하기 (e.g. prescriber signatrue missing script 이면 Legal point => Can't legally dispense this medicine. 하지만 내가 이 환자를 그냥 돌려보낸다면 생기는 Ethical point도 고려해야함. 결과적으로는 'I can't dispense this medicine because it's not legal to dispense the script that's prescriber signature missing, but since this medicine is for your severe pain, I recommend to see you another doctor for another new rx 이런식으로)
- 고려할 것들 => Code of Conduct for Registered Health Practitioners, Code of Ethics, Professional Practice Standards
- 이걸 다 암기하고 있어야하는 건 아니지만 읽고 익숙해지기. 특히 PBA registration standards에는 약사로써 report 의무 상황들이 있으니 꼭 읽어보기 (e.g. What if I get a forged script, what do I have to do)
- legal/Illegal이 가장 우선순위에 있다는 거 명심

3) Part C : Problem Solving and Communication

- Reference 사용 가능
- 20 minutes (history taking + finding problem + calling the doctor + suggesting alternative)
- 환자 케이스에서 반드시 문제점이 있으니 이걸 찾고 의사한테 전화해서 해결책까지 제시해야함 
- 문제점이 무조건 Drug-Drug interaction 일거라고 결론 내지 말고, 다른 문제점 가능성도 고려해야 함 (경미한 DD interaction의 경우 추후 모니터링 하면 되는 경우도 있음)
-eMims에만 의존하지 말기. eMIMS는 모든 drug-drug interaction 가능성을 빠르게 보여줘서 좋지만 리스트업은 10개 되는데, 막상 환자에게 치명적인 drug-drug interaction은 없을 수도 있음 (실제로 인턴 친구가 시험장에서 eMIMS만 보고 drug-drug interaction이 8개 정도 나와서 의사한테 전화했는데 다 '아 그거 이미 알고 있고 그냥 모니터링 잘 하면되' 라고 다 퇴짜 맞고 시험 떨어짐. 실제 답은 AMH precaution 에 해당한 dosing adjustment 였음)
 
 
- Dispensing history 체크 잘하고 (only history you’ve done in your pharmacy 만 생각하면 안됨, make sure ask if patient taking other medications e.g. warfarin)
- common oral exam errors: Not confirming all medications/OTC meds/CAM, Not confirming medical history and management status, Assuming drug-drug interaction, Not addressing the primary/main issue (prioritisation is essential), Not listening to examiner’s cue, Not providing final recommendation and counselling points

워크샵 후기 : 자괴감=> 자신감 하락 =>걱정 불안 => 심한욕 (특히 Part C, 이럴거면 의사를 ㅎ ㅏ..왜 약사가 이렇게깢...) =>해탈 => 원상복귀 (현실 망각하고 또 노는 중) => 미래의 나를 믿는다
